Your clinician does not need to search through every number you have ever recorded. Put recent changes, persistent concerns, and the questions you want answered where they can be seen first.
Include the treatment record
- Medication names and recorded prescribed doses
- Dates, changes, missed or late records
- Symptoms with severity and timing
- Weight trend if relevant to the appointment
- Short notes about meaningful routine changes
Lead with what changed
A short summary of the last few weeks is usually easier to review than a long diary. Put persistent symptoms, medication changes, and your most important questions near the beginning.
